Include equipment used, such as a hoist or wheelchair.
For example uses AAC, limited verbal communication, hearing or vision needs.
Are there any behaviours of concern?
Is there a behaviour support plan in place?
Are any regulated restrictive practices in use?
For example epilepsy, diabetes, mealtime plan, medication prompts.
Current or past involvement with the justice system?This never stops us supporting someone. It helps us match the right worker.

Who can make a referral?

Support coordinators, Local Area Coordinators, plan managers, health professionals, families and participants themselves can all refer. We always confirm the participant agrees to the referral before sharing their details with workers.

What information do you need?

The participant's name, contact details, location, the supports and hours needed, any preferences about language, culture or gender, relevant risks or plans, and how the plan is funded. Send what you have and we will follow up for the rest.

What happens after I refer?

We contact you to confirm details, then send worker profiles for the participant to review. Once a worker is chosen, we arrange a meet and greet and a service agreement.
